HELP! Survey frenzy

I need a little help from all of you. I'm currently working on my Honours Thesis on the subject of Recommend-A-Friend programs and would need 1000+ respondents for this really short survey. Approx 1 min.

You can do it multiple times as well because it works by randomization. Just be sure you don't repeat for the same company/service option. Please help me forward to your friends and family too!
